Our office policy respects the patient’s rights and choices. It permits the patient or any individual responsible for payment to refuse payment, cancel payment, or seek reimbursement for services or treatment that is preferred as a result of responding to this advertisement for complimentary services within a 72-hour timeframe. Due to federal regulations, this policy does not apply to Medicare, Medicaid, or other federal insurance programs. It is the patient’s responsibility to determine whether their insurance includes such a policy.

**General Release:** In exchange for adequate consideration, such as the retraction or correction of Content from Stewart Chiropractic, the claimant (hereinafter referred to as “Releaser”) hereby releases Stewart Chiropractic from any and all actions, claims, or demands related to any alleged misrepresentation or content discrepancies. This release extends to all claims that the Releaser may have had, currently holds, or may hereafter possess against Stewart Chiropractic.
