Manual Therapy (Myofascial/Cupping/ Gua Sha): $55 per 15 minutes
Rehab & Therapeutic Exercise: $50 – $60 per 15 mins
Zone 1 (Under 20 mins from base of operations): $40
Zone 2 (Over 20 mins): $75+
Cameron Stewart, D.C., LLC, is an out-of-network provider for all insurance carriers. This means we do not bill insurance companies directly. We operate on a fee-for-service model, and payment is due at the time of your appointment. We accept cash, local checks, and major credit cards.
Many insurance plans have “Out-of-Network” benefits that may cover part of your care. If your plan covers out-of-network chiropractic care, we will provide you with a “Superbill” (a detailed medical receipt containing all required diagnosis and procedure codes) upon request. You can submit this to your insurance carrier to seek direct reimbursement.
A Superbill is a specialized document used by medical practitioners that allows patients to request reimbursement directly from their health insurance companies. Please Note: A Superbill does not guarantee payment. Each insurance plan is unique. We recommend contacting your provider before your first visit to confirm your coverage (see the script below).
To request reimbursement, you typically need to mail or upload the following to your insurance company’s claims department:
The Superbill provided by Cameron Stewart, D.C., LLC
A Copy of your Insurance ID card (front and back).
A Claim Form (usually available on your insurer’s website).
Tip: Always keep copies of these documents for your records.
We chose to be an out-of-network provider to ensure that your care is determined by medical necessity, not insurance policy limitations. Insurance contracts often dictate which therapies can be performed, how long a visit can last, and how many visits are allowed—regardless of what you actually need. By remaining independent, we can provide longer appointment times (concierge services), personalized treatment plans, and one-on-one attention without administrative interference.
Call the customer service number on the back of your insurance card and ask the following questions:
“Do I have out-of-network benefits for Chiropractic care?"
“What is my deductible, and has it been met?"
“What percentage of the visit fee is reimbursed after the deductible is met?"
“Is a referral or pre-authorization required?"
“How do I submit a Superbill for reimbursement?"
Please keep in mind that if you choose to submit a Superbill for reimbursement, you are authorizing the release of your medical information to your insurance company. This includes your medical diagnosis (ICD-10 codes) and treatment details, which will become part of your permanent record with the insurer.
We value your time and the time of our other patients. Because we provide dedicated one-on-one care and travel to your location, late cancellations significantly impact our schedule.
24-Hour Notice: To cancel or reschedule, please get in touch with us at least 24 hours in advance.
Late Cancellation Fee: Appointments cancelled with less than 24 hours’ notice will incur a $50 Cancellation Fee.
No-Show Fee: If you fail to attend your scheduled appointment without prior notice (“No-Show”), you will be charged 100% of the visit fee.
New Patient No-Shows: New patients who fail to appear for their initial evaluation will be billed a $50 administrative fee and may be required to prepay for future appointments.
Exceptions are made for genuine emergencies at the provider’s discretion. All outstanding fees must be paid before future appointments can be scheduled.
